Role Overview

  • As part of a mobile, multidisciplinary ACT team, the RN will provide comprehensive nursing care and support services to individuals with serious mental illness in community-based settings.

Key Responsibilities

  • Conduct physical and health assessments
  • Monitor ongoing health status and coordinate care
  • Administer IM injections and manage medications as prescribed
  • Provide psychiatric and substance use-related nursing support
  • Deliver health education, daily living skills training, and medication management
  • Coordinate medical treatment with primary care and community providers
  • Support individuals in obtaining stable housing and community resources

Qualifications

  • Active Registered Nurse (RN) license in New York State
  • Strong communication and interpersonal skills
  • Comfortable working independently in the community
  • Proficiency with basic office software and internet research
